Preparing for Your Next Doctor’s Visit

The National Institute on Aging wants everyone to know asking questions at medical appointments is a good thing but remember, a doctor only has so much time allocated to each patient so come prepared. Their advice includes asking for clarification whenever the doctor uses a term to describe a medical condition you are unfamiliar with. Also, be sure you understand how and when to take a new medication. For example if it should be taken with food, does that mean before, during or after a meal?

If your physician wants you to have a test done, find out why and what sort of preparation it will entail. Ask if there are any side effects or danger getting tested, and how long it will take to get the results. You may want to request a copy of the results, and if a specialist is doing the test, make sure they send the results to your physician.  

Once results are in, your doctor will most likely make a diagnosis based on your symptoms, your physical exam, lab work and medical tests. Ask your doctor to describe the condition, how serious it is, if it is a long-term disease and/or curable, and what is the best course of treatment. If it includes medications, ask why you are taking it, the side effects and how long you will be on it. 

If the doctor recommends surgery, ask why he/she is recommending it, the risks and benefits, and if there are alternatives to surgery. How expensive is the procedure, will you be hospitalized and how long is the recovery period? February is American Heart Month

The CDC says February is American Heart Month, making it the time to focus on cardiovascular disease. Experts say this includes shining a light on hypertension (high blood pressure), a leading risk factor for heart disease and stroke. The NIH National Heart, Lung and Blood Institute’s National Wear Red Day® is on the first Friday in February. The campaign encourages people to show their support by wearing red and encouraging family, friends and co-workers to do the same. 

Focusing on heart disease is important as it is the leading cause of death in the US for both men and women; and the awareness campaign helps people to realize it is largely preventable. Experts stress that every adult should be aware of their risk and know the healthy choices to make to reduce their chance of heart disease including coronary heart disease, which is the primary cause of heart attacks. 

The NIH explains your risk of heart disease is higher if you:

  • Have high blood pressure

  • Have high blood cholesterol

  • Are overweight or obese

  • Have prediabetes or diabetes

  • Smoke

  • Do not get regular physical activity

  • Have a family history of early heart disease (your father or brother was diagnosed before age 55, or your mother or sister was diagnosed before age 65)

  • Have a history of preeclampsia (a sudden rise in blood pressure and too much protein in the urine during pregnancy)

  • Have unhealthy eating behaviors

  • Are older (age 55 or older for women or age 45 or older for men)

Experts say for each risk you have, the higher your overall risk is for heart disease. While some risks cannot be helped such as your gender, age and family history, it is important to discuss with your healthcare provider what you can do to reduce your risk. Also the NIH says it can be helpful to join #OurHearts in Action, a group designed to encourage and motivate heart-healthy behaviors together.

Improve Your Balance with Exercises

The Centers for Disease Control and Prevention’s latest statistics show that one in four Americans aged 65 years and older falls each year.  Falls are the leading cause of fatal and nonfatal injuries among older adults, and the number of falls will continue to increase as America’s baby boomers grow older.

While these are worrisome facts, experts say there are some simple exercises seniors can do to help prevent falls and maintain their independence. Remember a common injury caused by an elderly person falling is a hip fracture that can seriously impact their life.  

Most balance exercises are easy to learn and do, do not require special equipment or a trip to the gym, and can be done anywhere.  The Mayo Clinic explains that while many exercises including walking can help maintain good balance, there are specific exercises targeted at improving a senior’s balance and stability.  They do caution however that if you have severe balance problems or an orthopedic condition, to get your doctor's okay before starting balance exercises.

Examples of balance exercises include: standing on one foot, walking heel to toe, back leg raises, and side leg raises.  To do the standing on one foot exercise, stand behind a sturdy chair; gripping for balance, lift one foot off the floor.  Hold for 10 seconds and then repeat with the opposite leg.  Repeat the exercise several times and increase the repetition as your strength increases.   

Walking heel to toe is done by positioning the heel of one foot in front of the toes of the other foot.  Your heel and toes should be touching.  Now select a spot ahead of you and focus on it to help keep you steady as you walk.  Try to step forward for as many as 20 steps.  Be sure to ask your healthcare provider for other balance exercises.

The Center for Disease Control & Prevention (CDC) reports millions of older adults fall each year—resulting in more than 34,000 deaths in 2019. Of those falls, 3 million seniors were treated in emergency departments for fall injuries; and one out of every five falls causes an injury, such as broken bones or a head injury. With these alarming statistics, experts say it is important for seniors to know their risk for a fall including whether they are taking medications associated with an increased risk. 

Examples of medications that could lead to a fall include those that suppress the central nervous system as they reduce alertness and slow reactions and movements. Some of these types of drugs include anti-anxiety medications like diazepam (Valium) and lorazepam (Ativan). Additionally, antihistamines can cause drowsiness and are used in over-the-counter sleep aids like Nyquil and Sominex, or combined with acetaminophen in products like Tylenol PM.  

If you or a loved one takes medication to treat an overactive bladder or tricyclic antidepressants to help lessen chronic pain, they too can affect a person’s balance. Other drugs that increase the risk of a fall are opioids such as codeine, hydrocodone and oxycodone.

Your first step to lessen your risk should be to talk to your doctor to see if you are taking any medication that could increase your possibility of a fall. Furthermore, experts say adjusting or making a change in medication is the easiest thing to do to help prevent a fall. If it is determined you are taking a medication known to cause light-headedness, dizziness, sudden drop in blood pressure, or chronically low blood pressure or hypoglycemia (low blood sugar), you and your healthcare team can regularly re-evaluate your need and dosage. 

Another way to mitigate your risk for a fall is to do exercises designed to improve leg strength and balance. In addition, be sure to get an annual eye exam and update your glasses as needed. Lastly check your home for trip hazards (keep clutter out of hallways and stairways) and install grab bars and better lighting where needed.

Go Slow When Starting or Getting Back into Exercising

Doctors caution everyone, especially seniors, to go slow when starting or getting back into an exercise routine; and before you begin, to consult with your healthcare provider. After all you want to get back into shape, and causing injury or harm to yourself, is certain to delay or prevent this progress. 

Moreover, doctors reiterate that while individuals extremely out-of-shape will benefit the most from regular exercise, they are also more at risk for sudden heart problems resulting from such a workout. Dr. Martha Gulati, editor-in-chief of CardioSmart, the American College of Cardiology's patient education initiative, says it is important to "pace yourself, don't do too much too soon and make sure you give your body time to rest between workouts." 

One way to exercise safely is to pay attention to your heart rate. If you notice it is not coming down as your intensity decreases while resting or is beating irregularly, you need to stop. Dr. Gulati warns chest pains are never normal nor is nausea, vomiting or extreme sweating and if you experience symptoms, call 911. In addition, neither dizziness nor extreme shortness of breath is normal but rather a signal to stop and consult with your doctor. Cramps may be a sign of dehydration or low potassium levels and should not be ignored. Care should also be taken in individuals with atrial fibrillation, which is an irregular heartbeat, and seek emergency medical attention if such an event occurs.

Social Isolation and Loneliness are Linked to Poor Health

According to the Centers for Disease Control & Prevention (CDC), statistics show seniors who report feelings of loneliness are at higher risk for several diseases including dementia and are more likely to experience premature death. The National Academies of Sciences, Engineering, and Medicine (NASEM) says “one-third of adults aged 45 and older feel lonely, and nearly one-fourth of adults aged 65 and older are considered to be socially isolated.” Social isolation is a lack of social connections and a result of a person living alone, having a spouse and/or other supportive friends or relatives die, or suffering from chronic illness. Other related problems include a lack of transportation, mobility impairment and untreated hearing loss, which all limit the senior’s opportunity to engage with others. 

AARP Research surveyed adults 45 and older regarding their feelings of loneliness. Of those respondents who rated their health as excellent, only 25 percent said they were lonely. Among participants who said their health was poor, 55 percent reported feeling lonely. The CDC also says studies find social isolation or loneliness is linked to a 29% increased risk of heart disease and a 32% increased risk of stroke as well as a 50% percent increased risk of dementia.
